Dear Patients:

We deeply regret to inform you that we will be closing Clinic for the Cities at First Baptist Church in Wylie, First Baptist Church in Allen, and Clinic for the Cities in St. Thomas Catholic Church in Saginaw effective September 30, 2019. Unfortunately, after that date we will be unable to continue providing healthcare services or renew any prescriptions. This has not been an easy decision for us, as caring for you has been a great source of satisfaction and joy ever since we started providing this free service. Dr. Eugene Gicheru will be available by phone at 972-505-4003 through December 15th, 2019 to answer any questions about the transition of your healthcare but unfortunately, he will be unable to provide prescriptions or medical advice. Please arrange to place yourself under the care of another healthcare provider as soon as possible to ensure continuity of care. 

On October 8, 2019 all patient records will be sent to VeriTrust in Houston, Texas to be the custodian of medical records. After this date please start requesting your medical records from VeriTrust. To request your medical records please visit the site below to sign and return the medical release form. Authorized release of informationwill be provided during this seven (7) year period for adults and until a child turns 21 years of age. A copy of the release form is availableat: http://veritrust.net/client-support/medical-records-request/. The completed form can be submitted via fax, 281-758-8316, emailed to medicalrecords@veritrust.net or mailed to: Medical Records Fulfillment , P.O. Box 550748HoustoT77255. If there is any trouble with submitting, please call 713-263- 9000 and ask for the Medical Records department.
